The volunteer hereby freely, voluntarily and without duress executes this release under the following terms:

1. Assumption of Risk. The volunteer understands that the activities/work may be hazardous to the Volunteer, including, but not limited to lifting, loading and unloading, and other warehouse activities. Volunteer hereby expressly and specifically assumes the risk of injury or harm and releases The Food Bank of Northeast Louisiana.
2. Release and Waiver. Volunteer does hereby release and forever discharge and hold harmless The Food Bank of Northeast Louisiana and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from Volunteer’s activities with The Food Bank of Northeast Louisiana. Volunteer understands that this release discharges The Food Bank of Northeast Louisiana from any liability or claim with respect to any bodily injury, personal injury, illness, death or property damage that may result from Volunteer’s activities. Volunteer understands that The Food Bank of Northeast Louisiana does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health, or disability insurance in the event of injury or illness.
3. Medical Treatment. Volunteer does hereby release and forever discharge The Food Bank of Northeast Louisiana from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with the Volunteer’s activities with The Food Bank of Northeast Louisiana.
4. Insurance. The volunteer understands that, except as otherwise agreed to by The Food Bank of Northeast Louisiana in writing, The Food Bank of Northeast Louisiana does not carry or maintain health, medical or disability insurance coverage for any Volunteer. Each Volunteer is expected and encouraged to obtain their own medical or health insurance.
5. Other.Volunteer expressly agrees that this Release is intended to be as broad and inclusive as permitted by the laws of Louisiana, and that this Release shall be governed by and interpreted in accordance with the laws of Louisiana. Volunteer agrees that in the event that any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this release which shall continue to be enforceable.
6. Use of Image. Volunteers grant permission to the rights of their image and likeness as recorded on audio or video tape without payment or any other consideration, and understand that their image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein his/her likeness appears. Additionally, event attendees and participants waive any right to royalties or other compensation arising or related to the use of their image or recording. It is understood that this material may be used in diverse media relations uses within an unrestricted geographic area.

SAFETY GUIDELINES:

The Food Bank of Northeast Louisiana follows Best Practices to promote food and personal safety. The following guidelines protect your safety and ensure a safe food supply chain at the Food Bank.

Food Safety

In order to prevent contamination of food products, no persons with cold symptoms, flu, boils, sores, infected wounds or any other infections or communicable diseases are permitted to contact food.
Volunteers must wash hands before beginning shift, and after eating, drinking, using the restroom, and otherwise soiling hands.
No food is permitted in the warehouse areas of the facility, including gum and candy. Any spill or improperly disposed of food or drink product can contaminate inventory food products and be attractants for insect pests and rodents. Bottled water is approved and available.
No one is allowed to consume or remove from the premises any donated or purchased food or other non­food products.
Report any glass breakage to a staff member immediately. Staff members are trained in the proper procedure to clean up glass breakage in order to prevent contamination of inventory food products.
The FDA has identified the top eight allergens as wheat, milk, eggs, fish, seafood, peanuts, tree nuts and soy. Spills of any products containing these allergens need to be reported immediately to a staff member. Staff members are trained in the proper procedure to clean up food allergens to prevent contamination of inventory food products.

Personal Safety

Volunteers must check-­in at the front desk on arrival.
Volunteers are assigned to specific work areas in the warehouse. For personal safety and accountability, volunteers must remain in their assigned work area until their shift ends or they are reassigned to another project. Volunteers should not walk throughout the warehouse without being escorted by Food Bank staff members.
Open-­toed shoes are not permitted in the warehouse in order to protect you from powered equipment and the movement of products on pallets.
Be aware of forklifts and pallet jacks. Pay attention when pallets are being moved.
